Twmpath Ltd was registered in 2024, it comprises of highly qualified and experienced staff members combining over 30 years of experience working with young people within social care. We offer quality accommodation, with tailored support packages for care experienced young people.
Twmpath was derived from a wealth of knowledge within children’s social care and is operated by an experienced qualified manager.
Twmpath is fully encompassing of the young persons wishes whilst drawing on the details that matter. Our dedicated team of professionals have comprehensive knowledge and training relevant to supporting care experienced young people within the social care setting.
Our Mission
To be a sector leading provision in Wales, offering outstanding support and accommodation for care experienced young people aged 16+ . We will deliver this through tailored support plans, empowering young people to independence to lead happy and healthy lives.
Theory Informed Practice
Twmpath believes that a holistic multi collaborative approach to care and support is vital in the welfare and development of young people. Working in this way, it enables and promotes the young person to achieve their outcomes and aspirations. We are a person-centred organisation that provides quality support and accommodation, whilst enabling local authorities to reduce the spend on social care. We work alongside young people using a strength based approach, to provide an environment that is safe, structured and an engaging experience.
Twmpath adopts a trauma informed approach, to ensure a safe physical and emotional environment. All encompassing an ethos that build upon the strengths of the young people. It’s an approach whereby staff work in partnership with young people and professionals, focusing on the strengths of the individual.
We also utilise Systemic Practice, which is embedded in all of our work with the young people we support. We work on the basis that all young people should have a say in the care and support they receive, thus a large emphasis is placed on young persons objectives, and is supported weekly though a positive reward system. The tailored support plan feeds into the young people’s pathways plans so outcomes can be identified and met.
Collaborative Approach
All of our packages are developed collaboratively with the young person and professionals with an implementation of a reduction plan. This approach allows the young person to acquire more autonomy and promote their independence in a methodical, secure and monitored way over a prolonged period of time.
Young Persons Wishes & Feelings
Our mission is to enhance the quality of life for young people by offering comprehensive and tailored support that promotes their physical, emotional, and social well-being. We believe in treating every young person with the utmost respect, dignity, and compassion, ensuring they experience a sense of belonging and security within their community.
We invite the young person to visit their potential home with their social worker or support staff, so they can establish a sense of what the property feels and looks like. We promote a transition period where the young person can begin building a relationship with staff and have input to what their home looks like.
A pre placement document will be completed with the young person to ensure we have all the additional information needed to best support them.
All young people will be provided with essential information about the accommodation, including a weekly support timetable detailing the days, times, and staff members who will be available to assist them. Moreover, they will be given a welcome pack containing useful resources such as recipes, budgeting plans, and other relevant information to help them settle into their new home.
All support sessions are recorded electronically by support staff on our own management system. This will be used to collaborate weekly support records for social workers and to ensure support plans are up to date.
